All divers must complete a Queensland Medical Questionnaire. Click here to download it. Please be aware that certain medical conditions (eg asthma, heart disease, diabetes) and many prescription medication (excluding the oral contraceptive pill) may preclude some people from diving. For advice please contact our office directly. Final decisions rest with our qualified dive instructors who consider your safety – our highest priority.
